Cullwick is reported to have suffered a heart attack while diving at Leicestershire's Stoney Cove inland site. Scuba Trust has said that Cullwick was 'an inspiration to us all, a father-figure to many, and a good friend to a multitude'.
It added: 'The Scuba Trust will sorely miss his presence. His wit, his smile, his ingenuity in devising aids for many disabled divers, his ability to take as well as give jokes, his support of the Trust, his help with old and young alike, his care in making sure all were safe and happy both above and below water - all this made him a very, very special human being.
'Our hearts go out to Lew's widow, Gill, and we send her all our support and love.'
